In this Astute Clinician Lecture, Swee Lay Thein uses two clinical vignettes to show how observations of phenotype that challenge expectations based on genotype can open new therapeutic directions in inherited red-cell disorders. One vignette traces a path from clinical observation to gene therapy; the other examines an approach to increasing red-cell energy through enhanced metabolism and ATP production. The lecture explains how differences between observed phenotype and expected genotype generate new hypotheses, describes the clinical observations and scientific reasoning behind the gene-therapy approach, and sets out the rationale for enhancing red-cell metabolism as a therapeutic strategy, highlighting the role of bedside observations in shaping scientific questions and advancing therapies for sickle cell disease and other inherited red-cell disorders.
