IRISIN AND EXTRACELLULAR VESICLE–ASSOCIATED BIOCHEMICAL MARKERS DEFINE THE SIGNATURE OF INTENSIVE PHYSICAL ACTIVITY IN PARKINSON’S DISEASE

PD patients were recruited into an active group (n=24), undergoing 3 months of high-intensity treadmill training, or a sedentary group (n=20). Plasma EVs were isolated and quantified. EV cargo (inflammatory markers, alpha-synuclein, and BDNF) was analyzed by Western blot and ELISA.
Plasma irisin was measured by ELISA in PD patients (sedentary n=30, endurance-trained n=15) and matched healthy controls (sedentary n=30, endurance-trained n=15).
At baseline, EV concentration correlated with UPDRS III (p = 0.016), MoCA (p = 0.04), and age (p = 0.041). Alpha-synuclein correlated with disease duration (p = 0.046). EV concentration, alpha-synuclein and BDNF did not change between baseline and follow-up. In the PD-active group, a negative correlation emerged between ∆EV concentration and disease duration (p = 0.0043).
Resting irisin levels were significantly lower in active versus sedentary patients (p = 0.003).
Preliminary data suggest biochemical correlations with both motor and cognitive features in PD, as well as an effect of exercise on irisin levels. Further analyses are ongoing.
