Abstract

Dementia with Lewy bodies (DLB) is the second most common neurodegenerative dementia after Alzheimer’s disease (AD), yet its clinical diagnosis remains challenging due to overlapping symptoms and pathology. Reliable peripheral biomarkers are therefore urgently needed, particularly at prodromal stages. Building on previous transcriptomic evidence indicating enrichment of neuroinflammatory and neurodegeneration-related pathways in DLB platelets, this study aimed to characterize platelet protein immune signatures and activation states in prodromal DLB (pDLB) compared with DLB, AD, and cognitively unimpaired individuals (CU). Platelet lysates from 30 DLB, 60 pDLB, 51 AD, and 64 CU participants were analyzed using three independent custom Luminex multiplex protein assays targeting thirteen pro- and anti-inflammatory immune modulators. Selected findings were validated by enzyme-linked immunosorbent assays (ELISAs). Platelet activation and reactivity were assessed by spectral flow cytometry (AURORA) through measurement of CD62P and PAC-1 expression at baseline and following stimulation. Twelve of the thirteen analyzed immune-modulator proteins, including IFN gamma, IL-1 beta, IL-6, IL-8, IL-10, IP-10, TNF alpha, RAGE, and SDF-1 alpha, were significantly increased in pDLB platelets compared with AD and CU, with five markers also elevated relative to DLB. Sex- and age-at-onset–dependent differences were observed. Functionally, pDLB platelets displayed reduced baseline activation but heightened reactivity upon stimulation. These findings indicate that platelets in early DLB are enriched in immune and neurodegeneration-associated proteins while showing restrained basal release, suggesting altered immune containment. IFN gamma, IL-6, IL-8, IP-10, and RAGE emerge as promising peripheral biomarkers for prodromal DLB, warranting further validation.
